Standard EMS Transport Pricing

Rates are based on NY State EMS billing guidelines. Most transports are covered by Medicaid, Medicare, or private insurance when medically necessary.

BLS — Basic Life Support

Certified EMT crew, non-invasive monitoring, standard medical equipment. Covers hospital discharges, dialysis transport, nursing home transfers.

Typically Covered By

Medicaid (with prior auth)Medicare Part BMost private insurance

Most common transport level. Medicaid typically covers 100% for medically necessary trips.

ALS — Advanced Life Support (Level 1)

NY State–licensed Paramedic, cardiac monitoring, IV access, medication administration. Required for higher-acuity patients.

Typically Covered By

Medicare Part BMedicaid (with auth)Most private insurance

Medicare reimburses ALS1 at a higher rate than BLS. Prior authorization often required for Medicaid.

ALS — Advanced Life Support (Level 2)

ALS2 applies when 3+ medications are administered or advanced airway management is performed during transport.

Typically Covered By

Medicare Part BPrivate insurance (case-by-case)Medicaid (limited)

ALS2 billing requires detailed documentation of interventions performed during transport.

CCT — Critical Care Transport

Specialized critical care crew for ventilator-dependent, hemodynamically unstable, or ICU-level patients. Includes advanced monitoring and intervention.

Typically Covered By

Private insurance (pre-auth required)Medicare (limited)Self-pay / facility billing

CCT is rarely covered by Medicaid. Most often billed to the receiving facility or private insurance.
